In this context, Transition refers to the process of maturing and transitioning into adulthood, which is both thrilling and difficult. It comes with changes and decisions which are numerous and that also necessitate careful consideration. We recognise that it can also be a stressful period for parents / carers of young adults / adults with mental and physical health needs, as well as educational and additional requirements.

The most effective method of guaranteeing a positive and seamless transition and obtaining the appropriate assistance and support is to be adequately informed and prepared for the journey. Moving / transitioning from children’s social care services to adult social care services can be an overwhelming and stressful experience for some. A Lot of young adults have a hard time with this, but that is exactly why we’re here to assist and provide the most appropriate support needed.

Our organisation serves to provide a transitional service between being under the care of children’s social care services and moving over to young adults’ social care services for Emotional Behavioural Difficulties (EBD) and challenging behaviours. In general, we support young adults between the ages of 18 and 25 years into older adults. Inclusively, we work on ensuring that the young adults / adults in our care have access to the resources they need to overcome their fears, anxieties, and emotional distress that they can encounter during this change.

Our transition support provides young adults and their families with clarity, stability, and reassurance during a critical life stage and change. We help individuals build confidence, develop independence, and access the right services at the right time, reducing anxiety and disruption during the move from children’s to adult social care. Through personalised, well-planned support, we promote emotional wellbeing, continuity of care, and positive long-term outcomes for each individual we support.
