
OUR MENTAL HEALTH SUPPORT SERVICES
Our mental health support is committed to listening to individuals and making them feel valued, protected, and actively supported. We work collaboratively with the local authorities in providing mental health support from 1:1 nursing with registered mental health nurses in the community ,1:1 care and support from our support workers. We also provide planned, supported transition that focuses on safety, stability, and restoring a person’s sense of belonging. We recognise that reintegration back into the community is not a single event but a process that requires coordination, compassion, and continuity of care. Our goal is long‑term recovery by building trust, strengthening relationships, and empowering the person to live safely and meaningfully.
OUR REINTEGRATION SUPPORT SERVICES INVOLVE:
Preparing for a safe and gradual transition
- ➤ Working collaboratively with the individual, hospital clinicians, community mental health teams, social care, and family or carers.
- ➤ Ensuring the individual understands their discharge plan, support network, and what to expect, where they have capacity.
- ➤ Identifying risks, strengths, and protective factors to ensure a well-informed, person-centred transition.
Re-establishing daily life in the community
- ➤ Supporting the individual to rebuild routines, relationships, and independence at a manageable pace.
- ➤ Assisting with reconnection to education, employment, hobbies, or community activities.
- ➤ Encouraging confidence, autonomy, and a renewed sense of purpose.
Ongoing emotional and practical support
- ➤ Maintaining continuity of care through coordination with community mental health services, GPs, and social care.
- ➤ Reducing stigma through open, supportive conversations that validate individual experiences.
Maintaining Safety and Stability
Ongoing monitoring and responsive support
- ➤ Monitoring wellbeing and responding promptly to changes in mood, behaviour, or identified risks.
- ➤ Ensuring individuals know how to access support, including crisis services when needed.
- ➤ Working within a multi-agency network to prevent isolation and promote resilience.
Our goal is long-term recovery through trust, meaningful relationships, and empowerment. We support individuals to live safely, confidently, and with purpose. No one should face difficulties alone — we stand together, offering consistent support, understanding, and hope.
