OUR complex CARE SERVICES
Our complex care service delivers specialist tailored support for individuals with multiple, long-term, or complex health conditions that require a higher level of care. Delivered by trained and experienced staff, support may include assistance with clinical needs, personal care, medication management, mobility, and daily living tasks. Care is planned in partnership with healthcare professionals, families, and multidisciplinary teams to ensure safe, coordinated, and person-centred support, enabling individuals to live with dignity, comfort, and the best possible quality of life in their familiar surroundings.
Our Core Principles
CLINICAL EXCELLENCE AND SAFETY
We deliver care that meets the highest standards of clinical governance. Our staff are trained annually and competency‑assessed to support complex health needs, including (where applicable) ventilation, enteral feeding, seizure management, medication administration, and other delegated healthcare tasks. We work closely with healthcare professionals to ensure safe, evidence‑based practice.
PERSON‑CENTRED PLANNING
Every individual receives a personalised care plan that reflects their strengths, preferences, routines, communication style, and long‑term goals. We recognise that complex needs do not define a person; however, their identity, relationships, and aspirations remain central to our approach.
SKILLED, CONSISTENT WORKFORCE
Our complex care team is carefully selected, trained, and supervised by registered nurses and professionals to deliver high‑acuity care with confidence and compassion. Staff receive ongoing clinical training, reflective supervision, and regular competency reviews to ensure they remain skilled, safe, and responsive.
MULTI‑DISCIPLINARY COLLABORATION
We work in partnership with NHS teams, community clinicians, therapists, social care professionals, and families to ensure coordinated, holistic support. We value shared expertise and maintain clear communication across all agencies involved in the person’s care
PROMOTING INDEPENDENCE AND QUALITY OF LIFE
Complex care is not only about meeting clinical needs, it is about enabling people to live meaningful, connected lives. We support individuals to participate in daily activities, pursue interests, and maintain relationships, adapting our approach to maximise independence wherever possible.
SAFEGUARDING AND RiISK MANAGEMENT
We uphold robust safeguarding practices and proactive risk management. Our approach balances safety with autonomy, ensuring that individuals are protected while still having opportunities to make choices and take positive risks.
RESPECT, DIGNITY AND PRIVACY
We recognise the sensitivity of providing intimate and clinical care within a person’s home. Our staff act with professionalism, discretion, and respect at all times, ensuring that the home remains a private, personal space.
RESPONSIVE, RELIABLE SUPPORT
We provide care that is dependable, flexible, and tailored to changing needs. Our service adapts quickly to clinical updates, equipment changes, or shifts in the person’s condition, ensuring continuity and stability.
THE BENEFITS
The benefits of complex care include access to highly tailored, specialist support that meets intricate medical, physical, or behavioural needs in a safe and coordinated way. Complex care promotes stability, reduces hospital admissions, and ensures continuity through consistent, skilled staff who understand each individual’s condition and routines. By delivering care in familiar surroundings, it supports dignity, comfort, and quality of life, while offering families reassurance that care is delivered collaboratively with healthcare professionals and adjusted as needs change.
